Re: When to use the Dual Employment Form versus Oustide Consulting Form 1/6/2010

Outside Consulting Form is a system form that is required for faculty members from one of the component universities who will be teaching at another campus outside the primary assignment.  For instance, a UH Main campus faculty member who is teaching at UHCL.

Dual Employment Form is a UH Downtown form that is required for any tenured/tenure track faculty member from UHD who is teaching on our campus.  It requires UHD Provost approval.

Re: Faculty Folders for Visiting Scholars with No Teaching Assignment 11/11/09

A full faculty folder is not required for these assignments.  All that is needed is:

1.  Offer letter and acceptance to appoint with effective and end dates that clearly states there is no salary to

      be paid for this appointment

2.  Memo of recommendation approved by the dean

3.  PAR to hire on Empl Rec 499

4.  Recent CV

5.  Approval of Hiring Form

6.  Non-Tenure Track Faculty Employment Acceptance

Re: Visiting Scholars without SSNs 11/11/09

Since these are courtesy appointments at $0 compensation, they can be loaded without an SSN.  To do this, a hard copy PAR on empl record 499 should be completed. If the assignment converts to a paid assignment later, then an SSN will be needed.

Re: Summer Appointments for Non-Tenure Track 8/25/09

There is not a special summer appointment form for NTT faculty like there is for tenure track.  The regular reappointment letter is used for summer.

Re: Faculty FML Requests 7/8/09

All faculty FML entries must be processed via paper PAR, not ePAR, and routed by the college directly to HR for processing along with the FML request.   The academic notice of leave of absence form must be completed separately, and submitted by the college to Academic Affairs.  HR provides Academic Affairs with a copy of the disposition letter for record-keeping, and to confirm that the leave is appropriate.
