Reserving a spot for the trip
Graduate Students Only. Program is now open for sign-ups.
To participate in the trip, send Professor Currie (dcurrie@uh.edu) an email requesting a spot in the program. Please include your full name and date of birth. He will email you a confirmation of your reservation if spots are available. You then have 5 business days to submit the materials listed below to Professor Currie (or the office manager) in 315 Melcher Hall to secure the reserved spot. The materials may be emailed to Prof. Currie in one PDF file. If he does not receive the sign-up materials within 5 business days your reservation will be lost and your spot will be opened up to other students.
Upon receipt of all completed “sign-up materials" a non-refundable $300 deposit fee will be charged to the student’s account within 14 days. Students are accepted into the program upon payment of the deposit fee.
The trip is only open to graduate students. Please ensure you have carefully read the entire China Study Abroad Web Site and the Course Syllabi (MANA 7353 and MANA 7397) before signing up.
SIGN-UP MATERIALS
To secure your space in the trip, students must submit the following materials to Professor Currie by emailing them as one pdf file or delivering them to the Management Department office in 315 Melcher Hall within 5 business days of reserving a spot.
Please note that some of the materials must be notarized:
- Submit completed form with all materials.
- View China Cover Page.
- Faculty-Led Student Information Sheet
- Learning Abroad Release and Indemnification Agreement Form.
- Learning Abroad Image Release Form.
- Proof of registration with Smart Traveler Enrollment Program (US citizens only)
- Create an account
- Register your trip with all details
- Print and submit a screen shot of your profile, which must include your name, destination country, and travel dates)
- Color photocopy of your passport or passport application. Passport must be valid and not expire within 6 months of travel.
- Form A-1: Health Disclosure and Medical History. You must have health insurance that covers travel abroad to participate.
- Form S-2: Student Conduct Agreement Form. Sign and date.
- Proof of international insurance coverage that includes health insurance/medical and emergency evacuation/repatriation.
Students are encouraged to email these forms as one pdf file to Professor Currie at dcurrie@uh.edu.
